Combat Positioning/Distance removed, Ranged/Avoid count as defensive Weapon Reach

Per the changelogs:

- The incremental Combat Positioning/Distance mechanic has been removed.
- Holding a ranged launcher (bow, sling, etc.) or being in Combat Avoid mode now count as having maximum Weapon Reach in terms of avoiding melee attacks. This is based on the same calculations as normal Weapon Reach, so Balance and Roundtime timing are still important.
- Changing of one's Combat Mode (Avoid or Engage) can no longer be performed during roundtime.

The incremental positioning mechanic just didn't work out as well in practice as it did on paper, and had too many unintended side-effects and wonky situations as far as determining just where people were in relation to one another. In particular, it largely removed any sense of danger when traveling through areas with nasty critters because one could just walk away before the baddies closed to melee range.

Giving ranged weapons an effective maximum Weapon Reach for extra chances to avoid melee attacks should help them keep a little edge by virtue of being ranged, and ranged attacks still completely bypass defensive weapon reach as they always have. Expect some focus on balance management moves for ranged combat next.

P.S. Charge-type attacks such as Tackle or Shield Charge reduce the chances of that maneuver being avoided by Ranged/Avoid.
