A line in the 'look' description noting if someone is (disguised) or (costumed) would be helpful - perhaps mentioning that their eyes etc. are hidden from view as well. When someone newly arrives, if you 'look' at them, it's not at all obvious they're disguised without reading and parsing/understand...

I think the blood cultist encounter outside Shadgard may be a little much. - You can just walk away from these! You might get tagged in the initial encounter, but you don't need to stay and fight. Citizens of Shadgard who want to aid their settlement during an attack can already do so. I'm less certain about Mistveil Keep's view of the matter. There are multiple channels one can speak to; for example, you might be on GRAY but they're speaking to SAPPHIRE - if the ESP message was hidden, you'd just assume that everyone on the GRAY network heard what he had to say.
